2.Print your rebate form
Enter your order number in the box above and we’ll create your rebate form. Print it out then fill it out – one form for each rebate.
3.Snail mail your form to:

4.Simple. Your pre-paid Visa© card should arrive within 6-8 weeks.

Must complete and submit the Rebate Form within the following time periods: If your order ships between January 25, 2024 and July 25, 2024, you have until August 22, 2024 to submit your rebate form. Forms must be postmarked by July 25, 2024 to be valid; If your order ships between July 26, 2024 and December 31, 2024, you have until January 22, 2025 to submit your rebate form. Forms must be postmarked by December 31, 2024 to be valid. Rebates must be mailed to Rebate Processing Center, P.O. Box 777, Sandy, UT 84091. Allow 6-8 weeks for delivery. No P.O. Boxes, only street or rural address are acceptable. Rebates may be combined with other offers, except as described in our Best Price Guarantee. However, rebates may not be combined with insurance benefits. Manufacturer rebates excluded. Fraudulent submissions could result in federal prosecution under the U.S. Mail Fraud Statutes (18 U.S. Code, Section 1341 and 1342). Not responsible for lost, late, or undeliverable responses.
